Our scheduled fees are provided below for your reference.

Please note we require 24 hours notice for cancellations to avoid a cancellation fee. We understand that sometimes circumstances occur where this is not possible, such as a child falling ill overnight, but notification is expected as early as possible. If you fail to attend without notification you will be invoiced a cancellation fee of $50 and any scheduled appointments will be cancelled until payment is received.

Referral Pathway

Chronic Disease Management (CDM) plan referral form – must be completed by a GP.

Rebate Amount

Aboriginal and Torres Strait Islander (ATSI) people who have had a health assessment.

Referral Pathway

A referral form for follow-up allied health services for clients of ATSI descent must be completed by a GP who has conducted a health check consistent with the ATSI Medicare health checks.

Eligibility Requirements

Your child must be assessed as eligible for funding by the NDIA and have an NDIS plan approved.

Tablelands Speech Pathology is able to see clients who are

  • “self-managed” or
  • “plan-managed”

We are unable to see clients whose plans are managed by the NDIA (or “agency-managed”)
